EAST:MK Primary Care Network has exciting opportunity for a g eneral medical practitioner into our practice team on a fixed term contract until March 2025 with the possibility of an extension. The role is offered on s essional basis, 6- 8 sessions per week between Monday and Saturday, on a rotation basis across the 4 surgeries - Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery that make up the PCN. Responsibilities of the role will include: Deliver high quality clinical care to the patients through face-to-face/online/telephone consultations, home visits, assessments, diagnoses, treatment plans. Manage acute and chronic conditions, safe prescribing, handle referrals and prescription reviews, maintain accurate clinical records. Chronic disease management: To deliver care plans for chronic conditions and manage long-term conditions. Preventative care and screening: Promote immunisations, screenings, and lifestyle interventions. Focus on early intervention, disease prevention, and health education. Referral and coordination of care: Refer patients to specialist services and coordinate care across primary, secondary, and tertiary sectors. Collaborate with multi-disciplinary teams to provide seamless care for complex cases. Health promotion: Engage in health promotion initiatives such as mental health awareness, smoking cessation, and weight management. Refer patients to relevant agencies and contribute to population health strategies aimed at reducing health inequalities. Public health and community engagement: Participate in public health initiatives to address social health determinants and inequalities. Collaborate with local services and community organisations to improve patient well-being. Quality improvement: Take part in audits, research, and quality improvement projects. Ensure adherence to appraisal and revalidation processes, promoting continuous professional development. Contribute to reviews of significant events and near-misses using structured approaches like root cause analysis. Foster a culture of collaborative working and continuous improvement across the PCN. Adhere to the highest personal and professional standards as set out by the GMC and to put individual patient care as your priority. Work with the PCN manager, practice managers and clinical director to support the development, delivery and review of the surgery action plan – including list growth, patient satisfaction levels, QOF, enhanced services, KPI targets, resource profile and cost management. Essential qualifications, skills, and experience: A GP whose name is included in the General Practitioner Register (GPR). Included in the list of medical practitioners maintained and published by NHS England in accordance with section 91 of the NHS Act 2006. Within 2 years of date of your certificate of completion of training(CCT), issued by the General Medical Council. Special interest in gynaecology /women’s health, paediatrics etc is preferred. Highly developed interpersonal skills and ability to lead key meetings. Excellent communication skills- written and verbal. Able to drive and have use of a car.
