This post will cover the Triage and referral role for the South West Locality, the successful candidate will be required to triage for the locality, appropriately managing referrals and delegating urgent visits to the staff within the locality. The post holder will also be expected to provide support and guidance for the staff within the Team/locality.

As a Community Nursing Sister/Charge Nurse you will work closely with the multi-disciplinary team including GPs, Practice Nurses and Specialist Nurses in order to support the service delivery having a high focus on quality outcomes and improving the patient experience.
Main duties of the job
The post holder work closely with multidisciplinary teams and will work with our partnership health and social colleagues to provide optimal care plans for our complex patients.
Main duties include but are not exclusive:
Triaging for the South West Locality
Allocation of visits
Maintaining the clinical competency matrix for staff
Reviewing referrals and schedules to ensure appropriate
Skill Mix on a day to day basis
In the absence of a DN Sister lead handover
Undertaking clinical skills when required whether in wound clinic or Home visits.

You will need:
