Responsibilities:
* Conduct comprehensive assessments and diagnose mental health conditions.
* Develop and implement personalized care plans with other healthcare professionals.
* Provide therapeutic interventions, including counseling, medication management, and crisis intervention.
* Regularly review and monitor patients’ progress, adjusting care plans as needed.
* Educate patients and their families about mental health conditions and treatments, offering support to manage their conditions.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
