Job Purpose Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical social prescribing link workers who give time to focus on what matters to me. This role specifically works with young people, taking a holistic approach connecting them to community groups and statutory services that provide emotional and practical support. As well as delivering direct patient services personally you will also be responsible for the line management of a some of the team of Young Peoples Social Prescribers. Together, as part of the YHPs team of health professionals you will take referrals from primary care, schools and a wide range of agencies to support the health and wellbeing of patients. You will support the Lead Children and Young Peoples social prescriber with the performance of the young peoples social prescribing team and alongside the Head of People and Projects, delivery of the social prescribing plan as determined by the organisation. Job Summary The Young Peoples Social Prescribing Service will work closely with GP practices across the Primary Care Network, working alongside young people aged eleven to seventeen, supporting them to tackle loneliness, isolation, anxiety and depression by connecting them into a broad range of services & interventions to improve their health and well-being. The work will be led by the wants and needs of the young people themselves and may need some flexibility in working hours to fit availability of the young person. To provide personalised support to patients, their families, and carers to take control of their health and wellbeing, live independently, improve their health outcomes, and maintain a healthy lifestyle. Develop trusting relationships by giving people time and focus on what matters to them. Working collaboratively with community groups providing support to maintain and build accessibility, sustainability, and capacity. Key Areas of Responsibility To provide line management for a team of Young Peoples Social Prescribers To support the Lead Children and Young Person's Social Prescriber and Head of People and Projects in the delivery of the Young Peoples Social Prescribing Service. To act as a point of contact for young people accessing the service. Operational Management Accept referrals and assess how far a patients health and wellbeing needs can be met by services and other opportunities in the community. Manage and prioritise your caseload reflecting your patients needs. Together produce a simple personalised care and support plan to address their health and wellbeing needs. To ensure young people are made aware of key processes e.g. complaints, information sharing, getting involved, and to make sure that they are asked about how they would prefer information. Following a patients journey, evaluate how far the actions in the care and support plan are meeting their health and wellbeing needs. Where required refer patients back to other health professionals within the PCN. Financial Management and Planning Ensure data is captured in the clinical systems and SNOMED codes are utilised. Gather data to build an evidence base about the benefits of social prescribing. Building capacity through effective leadership Undertake performance management discussions, coaching and appraisals for the individuals that make up the link worker team. Support the VCSE organisations and community groups to become more sustainable, sharing intelligence regarding gaps or issues identified in local provision. Ensure all VCSE organisations have appropriate safeguarding procedures in place and are genuine opportunities for patients to develop a sense of belonging, build knowledge, skills, and confidence. Sharing the knowledge and promoting the benefits with colleagues in the PCN on other services available in the local community and how these can be accessed. Promote the social prescribing services you offer to colleagues within your PCN and the community. Be visible. Management of Communication Liaise with appropriate statutory and voluntary sector agencies working in Education (including SEND), Public & Mental Health, Primary and Secondary care, Social Care and Youth Offending Teams and build up appropriate networks that promote timely access to support for young people. To work systemically, e.g. liaise with and include parent/carers, educators, health professionals etc. when appropriate and as agreed by the service users. To build strong working relationships with internal and external services and to promote the link worker/social prescribing model. Work collaboratively with local Voluntary, Community and Social Enterprise (VCSE) organisations and community groups to receive social prescribing referrals. Risk Management and Quality Assurance Discuss concerns with a relevant GP to ensure safeguarding procedures are followed appropriately. To attend multi-disciplinary team meetings (MDTs) as required. For your own personal and professional development, you will be expected to participate in monthly supervision with a health professional. To attend mandatory training and abide by policies and procedures on safeguarding, health and safety and equality and diversity. To attend appropriate continuing professional development and training events, team meeting and supervision sessions. To carry out, from time to time, any other appropriate duties in line with capabilities as directed by the line manager. Management of workload and personal development Act on own initiative in prioritising and managing workload delegating tasks appropriately and referring to Chief Operating Officer in respect of clinical, managerial, and administrative areas, as necessary. Maintain own personal and professional development as agreed at your annual review.
