Therehabilitation service works towards individuals recovery by using therecovery model and rehabilitative model of care and support with adults withsevere and enduring mental illness.
Main duties of the job
The purposeof the role is to provide day to day (shift) nursing leadership within arehabilitation inpatient service to support individuals to meet their agreedgoals for rehabilitation and secure ongoing placement so that we treat peoplein the least restrictive environment.
Therehabilitation practitioner will provide specialist nursing advice to informassessment (including risk assessment), care planning, treatment andreview.
They willlead the shift and ensure meaningful activity aimed at rehabilitation andimproving life skills is provided daily.They will ensure that the group programme is adhered to and that allstaff are meaningfully engaged with our service users throughout their shift.

Job description
Job responsibilities
Carry out duties as requiredon a daily basis, to include the following:
Ensure a high standard ofnursing input in the application of clinical procedures, including assessment,risk assessment, care planning, implementation and evaluation, medicationmanagement, physical investigations and implementation of the Recovery Approach.
To liaise with the wardmanager in ensuring the safe and effective delivery of care, in line withnational, local and Navigo policy, through supervision of junior staff andaudit processes.
To undertake incident/accidentreports, safeguarding, complaints, investigations to ensure the safe running ofthe ward.
Attendance at weekly meetings and effective presentation of clinical reports and information.
Liaison with other disciplines to ensure effective team working and consistent approaches to caredelivery.
Oversee the effective deliveryof the MDT process.
Oversee the correctimplementation of negotiated care plans.
Supervise the completion ofnursing reports by Key/Associate Nurses (,, HospitalManagers, and MDT) in keeping with specified and statutory timescales.
Attendance at six monthlypatient meetings.
Attendance at and/orHospital Managers Hearings in the absence of the Key/Associate Nurse.
Ensure effective management ofmedication through training delivery, and carry out audits to ensure adherenceto policy and guidelines.
Oversee Key Nurse liaison withexternal Care Co-ordinators and Commissioners.
Dissemination of action pointsfrom Clinical Governance meetings
Support the ward manager inconducting and addressing issues of complaints and investigations.
Liaise with the ward managerin addressing issues involving whistle blowing.
Liaise with Key/AssociateNurse and ward manager in establishing and maintaining therapeuticrelationships with service users family (where appropriate).
Ensure adequate staffing tocover unit requirements and liaise with ward manager to address any deficits,taking into consideration financial budgets.
Carry out assessment visits aspart of the, assessing potential patients and preparing assessment andtreatment reports.
Monitor, undertake and overseeward based risk assessments and management Plans.
Carry out and monitor annualappraisals of nursing staff and support workers, as directed by ward managerand ensure regular evaluation of PDRs.
Have direct line managementfor staff nurses, nursing associates and nursing assistants.
Undertake, monitor and ensuremonthly supervision of nursing staff.
Chairing monthly ward staffmeetings, ensuring delegation and implementation of action points.
ProvideMentorship/Preceptorship to newly qualified or newly appointed nursing staff(including nursing students, where applicable).
Liaise with ward manager inthe identification of external and in house training (including mandatorytraining) for specified staff.
Provide temporary cover forward manager in the event of their absence, as required by the HospitalManager.
Undertake interviewing of newward staff, at nursing and support level.
Attendance at the morningstaffing meeting in the absence of the ward manager.
Support the ward manager inall personal related issues.
Oversee and Co-ordinatemonthly audit of all office files and develop and implement action plans toaddress identified deficits.
Co-ordinate monthly clinicaudits and implement action points to address any identified deficits.
Monthly input of KPI data,including senior nurse report, training, supervisions, appraisals, revalidationand sickness monitoring.
Ensure service user financesare managed according to policy and assist with audit as required.
In conjunction with theClinical Administrator, monitor and ensure that legal processes anddocumentation are in line with MHA (1983) guidelines and policy andprocedures.
Support the Associate Directorof Nursing and Quality in the completion of contractual and commissionerrelating documentation.
Oversee the completion ofindividual weekly therapeutic timetable.
Monitor the implementation ofward structured day, including the specified hours of meaningful activity.
Review the completion ofmonthly audit of Recovery focused 1:1 Key Worker and Key Nurse discussion.
Monitor three-monthly reviewof Recovery Star by Key/Associate Nurse or Key Support Worker. Monitor monthlyreview of HoNoS. Monitor monthly patient file audits and oversee actionsrequired and monitor that they have been completed.
To drive forward owndevelopment plan through supervision, appraisal.

1. 18 months experience working within a relevant mental health service with service users with acute or severe and enduring mental health needs.
