We are looking for a caring, motivated, and friendly Community Nurse to join our team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care.

Main duties of the job
You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for ongoing professional development. We also actively encourage newly qualified nurses to join our team. In return, we offer a preceptorship programme and support professional development in a nurturing environment, allowing you to reach your full potential.
The team provides nursing care to patients within their own homes or chosen place of residence, offering an alternative to hospital admission, supporting timely discharge, and enabling people to realise their potential.
We are committed to clinical support, CPD & training. The post is covering 7 days, 8 am to 6 pm service.

Qualifications
* RGN NMC Registered
* Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
Experience
* Experience of working in a multi-disciplinary / multi-agency health/social care environment.
* Experience and knowledge of basic administration systems including Microsoft Office.
* Experience of successful partnership working arrangements with other services.
* Previous community experience.
Skills
* I.T. skills or willingness to undertake RIO Training.
* Up to date with Mandatory Training.
* Uses evidence-based practice.
* Works within own defined competencies.
* Awareness of changing healthcare factors influencing change.
* Demonstrates tact and diplomacy.
* Well-refined communication (verbal and written), interpersonal skills - ability to gain rapport at all levels.
