We require a skilled & experienced Band 6 Older Adult Community Mental Health Nurse to work within our Older Adult Community Mental Health Team based at Balladen House, Rawtenstall, overlooking the beautiful Rossendale Valley.
The service provides assessment, treatment and Interventions to service users across the Pennine & BLACKBURN with DARWEN footprint, who have been referred with a variety of mental health conditions within the community setting- including those identified with a degenerative organic illness & Dementia.
The team is multi-disciplinary led, and works closely with other mental health services and teams in providing a holistic healthcare model.
Potential candidates ideally should have the relevant Older Adult /Adult Mental Health community experience, and have proven clinical skills matching with that of a Community mental health service
We are looking for a candidate who has excellent knowledge and experience, and is able to demonstrate a positive attitude for change and to the care of the Older Adults- sat within the community setting.

Knowledge and skills around the coordination role is desirable in line with the MDT approaches and processes, models and initiatives, which are underpinned by clear crisis & care plans, incorporating risk assessment and management plans.
The ability to manage risk, liaise with & communicate effectively is essential for the role, and applicants should be able to demonstrate sound working knowledge around the current national and local drivers which underpin community transformation programmes.
Candidates will be able to demonstrate resilience and the ability to manage the demands of a busy community team. Be able to offer management and clinical supervision to band 5 staff, and act as a practice assessor to support student activity.
There is a robust supervision and personal development structure in place via the service manager and the successful candidate will have development opportunities in house and externally.
