Siam Surgery has an opportunity for a care co-ordinator to join our team as an ARRS role within Sudbury Town Primary Care Network. This position is for 32 hours per week worked over 4 days, hours and working pattern negotiable. The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients. The main duties of the role include: Working with GPs and other professionals within the practice to identify and manage a caseload of patients. Particularly working with vulnerable patient groups with long-term conditions that may require this extra support. This may include working with individuals, their carers, families and healthcare teams. Using in-depth knowledge of locally available support and services to develop care plans with patients, focusing on prevention. Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred. Helping patients prepare for, or follow-up after, clinical conversations. Supporting digital initiatives. Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date. Please see attached job description for further information. About the Candidate If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. The ideal applicant will have the following skills, experience and qualifications: Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential). Familiar with local resources and services, including how to access them. Motivated by helping people, with care and empathy. Able to deliver person-centred support in a non-judgmental way. Experience of completing holistic person-centred care planning assessments. Strong listening and communication skills.
