Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients. Work as an autonomous practitioner within the integrated team providing rapid and planned assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands on clinical skills and knowledge to treat, educate and support patients on a designated caseload and by working with patients and their carers to assess, plan and implement appropriate packages of care.
