The Norwich Community Therapy team are advertising for a friendly, flexible and conscientious Community Assistant Practitioner, band 4, to work in a supportive and nurturing environment.
Main duties of the job
The post holder will visit patients in their own homes and carry out a holistic assessment focusing on how they manage with mobility, transfers and other activities of daily living in their own environment. They will assess a variety of conditions and liaise with other health professionals working to enable the patient to be as independent for as long as possible.
Job responsibilities
The post holder will provide supportive exercise programmes and equipment to enable the patient to improve or maintain their mobility and prevent falls. The post holder will need to be able to drive and have their own car.
A supportive induction programme will be offered. The post holder will receive monthly supervision with a registered Occupational Therapist or Physiotherapist. Training is encouraged.

Qualifications
* oNVQ 3, or equivalent knowledge and experience
* oFoundation degree programme, or equivalent demonstrable knowledge and skills acquired through training and experience
* oGCSE education, Grade C or equivalent in English and Maths
* Experience of working in a health or social care setting
Experience
* Experience of working with people at the end of life
* Experience of working with people, in the statutory/voluntary/private sector.
* oExperience of working in a community health care setting.
