Job summary CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting To support the day-to-day running of the administration needs of the Frailty Hospital at Home Service and the Unplanned Care team and to act as a point of contact for professionals. To provide administrative support to the Frailty Hospital at Home Service and Unplanned Care team to enable them to meet client needs and undertake their role, delivering quality patient care. The post holder will be flexible in order to meet the requirements of the team's remit. To work as part of a team ensuring that the daily administration tasks are completed effectively and efficiently. To communicate with professionals and the general public with tact and sensitivity. To ensure that accurate details are exchanged with all relevant staff. To give support to patients, carers and staff where appropriate, acting as a supportive member of the teamat all times. Main duties of the job Carrying out routine entry of information and scanning of patient notes onto the SystmOne database. Register referrals for all Unplanned Care pathways - Frailty Hospital at Home Service, Discharge To Assess (DTA), Rapid Response and Early Intervention Vehicle (EIV). Complete capacity report on an ad hoc basis. To support in the monitoring of stationery cupboards and medical storeroom for stock and support in the ordering of service stationery and medical supplies when required. Forwarding discharge summaries, referrals and other letters to patients, general practitioners and other services. To carry out general office tasks such as sorting incoming mail, franking outgoing mail, photocopying and keeping the office tidy. To accurately record information on Trust IT systems and databasesTo answer incoming calls, dealing with enquiries in a helpful manner, proactively taking action where appropriate. To deal with enquiries, correspondence and information received by the service in conjunction with other staff.
