Job overview
The candidates will work closely with the local community teams and ensure early escalation of unwell patients. They will need to be motivated and innovative with strong leadership and communication skills to support the development of the Virtual Ward.
Main duties of the job
The successful candidate will provide the highest standard of individualised and holistic patient care, in conjunction with the interdisciplinary and multidisciplinary teams. The post holder will work within the NMC Code, trust policies, guidance, and procedures to ensure safe working practice.
Person specification
Experience
* At least three years relevant post-registration clinical practice experience
* Experience working in the care of people living with respiratory diseases
* Physical Examination qualification or working towards it
Skills
* Knowledge of common skills required to care for respiratory conditions e.g. chest clearance
* Excellent interpersonal and organisational skills
* Ability to work within set timeframes working to priorities and deadlines
* Ability to work as part of an effective multi-disciplinary team
* Ability to work independently and as part of a team
* Ability to prioritise clinical work effectively
* Can show a non-judgemental approach to patient care
* Ability to monitor the quality of own work and others
* Able to recognise own limits and work within those limits of competence
* Ability to use and maintain resources efficiently and effectively
Knowledge
* Is aware of how to identify and reduce risks with regards to health and wellbeing
* Demonstrates good understanding of various National Guidelines for COPD, Oxygen, and Pulmonary Rehabilitation
* Demonstrates good understanding of various National Guidelines for Asthma and NIV management
* Knowledge of Capillary Blood Gas analysis
Qualifications
* Chartered Physiotherapist

* Ability to undertake teaching and sharing knowledge
* Ability to support and supervise learners in practice such as student nurses, student therapists, and medical students
* Ability to communicate effectively at all levels across the hospital
* Ability to treat everyone with dignity and respect
* Can Drive and has a valid UK Driving License
