The post holder will Assess and manage patients with minor illnesses at the surgery and in local care homes providing effective treatment and advice. Provide health education and advice to patients, empowering them to make informed decisions about their care Advise and prescribe (if qualified to do so) on evidence-based treatment for minor illness conditions Make any necessary referrals to other members of the primary and secondary health care team as necessary and with clinical support from the supervising GP. Work autonomously and as part of a team to deliver patient care in line with best practice guidelines and clinical protocols The ideal candidate will support the practice team with chronic disease management, wound care, spirometry, health promotion, vaccination and screening activities. Maintain accurate patient records, ensuring confidentiality and compliance with all relevant healthcare regulation Participate in continuous professional development and keep up to date with changes in clinical practice
