Community Children's Practice Development Nurse/Diabetic Nurse
In consultation with the PDNS, provide specialist support and advice related to the development of services for children and young people with diabetes and be an integral member of the multidisciplinary diabetes team which provides support to children and young people in the acute and community setting. Provide advice, support, and participate in the education and development of staff working with children and young people with diabetes, including schools and community healthcare professionals within the scope of professional knowledge and specialty. Work collaboratively and cooperatively with colleagues to develop services and ensure a high quality of care.
Ongoing education of all staff in the service, trained and untrained staff.
Facilitating placement of learners of all types in the service.
To take charge of the service, ensuring the delivery of safe effective care.
Main duties of the job

Job responsibilities
In consultation with the PDNS, provide specialist diabetes education and support to children and young people and their carers within acute, community, and school settings.
In consultation with the PDNS, provide health promotion and clinical knowledge to patients, carers, and relatives.
In consultation with the diabetes service, identify when acceptable standards of care could not be achieved, identify reasons and inform PDNS.
