Grantham Crisis Resolution and Home Treatment Team (CRHT) are welcoming applications from experienced nurses/healthcare professionals with a passion for making a difference to the lives of those experiencing acute mental health crises. The team provides a swift response to people who are in the acute phase of mental illness, providing both assessment and treatment in the community, for Grantham and the surrounding areas., As a crisis service, we provide intensive support for adults experiencing an acute mental health crisis through assessment, care planning and home treatment.
Grantham Crisis team work a shift pattern of 3 long days per week, including weekends and bank holidays.
We are looking for applicants who are dynamic, passionate, and committed to achieving quality outcomes that will ensure above all else the needs of the patient are paramount in everything we do.
As a team we work hard to establish effective working relationships with patients, their families and carers including education and promoting individual rights, recognising and respecting their contributions to care planning, placing patient in the centre of their care. Referral on to the appropriate support, ie medical, mental health, physical health, third sector, well-being or social prescribing.
We work hard to facilitate and maintain good working relationships with our colleagues in other teams and work as one to ensure the patient remains at the heart of what we do.
