Salary: Up to £55,000.00 FTE per year (depending on level of experience, qualification and training)

We have an opportunity for a Senior Clinical Pharmacist to join their team of staff in the Pharmacy Department.
This is a part-time role for either 30 hours. The role holder will be required to cover a flexible shift pattern.
Duties of this role include:
* Provide expert clinical pharmacy input as part of the multidisciplinary team, conducting medicines reconciliation, clinical screening, and therapeutic drug monitoring.
* Optimise medicines use through evidence-based interventions, ensuring the safe and effective management of long-term and acute conditions.
* Support prescribing decisions through clinical assessment, risk evaluation, and formulary guidance.
* Lead on implementation of medicines optimisation strategies, ensuring prescribing is cost-effective, safe, and aligned with national/local guidelines.
* Contribute to the development and review of clinical guidelines, protocols, and policies.
* Monitor and report on medicines usage, compliance, and safety incidents, participating in root cause analysis and quality improvement.
Applicants should meet the following criteria:
* GPhC-registered Pharmacist.
* Significant post-registration experience in clinical pharmacy, preferably in a hospital setting.
* Strong clinical knowledge and experience in medicines optimisation.
* Excellent communication, leadership, and multidisciplinary collaboration skills.
* Experience in mentoring or supervising junior staff.
