We have a fantastic opportunity for a Day Care Team Leader to join our team based in Cheltenham on a permanent basis, working 40 hours per week (12.5-hour shifts over a 7 day shift pattern). You will also be included in the rota for on call.

KEY RESPONSIBILITIES
As a Care Team Leader, you will work as part of our team in a service for ex-offenders in the achievement of the Trust’s aims and objectives. You will also assist in the management of the Service.
* Assisting the on-site management team in supervising all care staff and monitoring the care given to our clients. Spend time with clients and monitor service standards.
* Conduct regular key working sessions with allocated clients and enable them to participate fully in the life of the service, including clients’ meetings. Helping clients achieve the aims of their support plan.
* To provide personal care to identified clients as required.
* To maintain a clear and calm manner whilst managing client risk through assessment, planning, client knowledge and de-escalation as required
* Helping induct, train, and supervise colleagues and junior members of staff in all aspects of their work in the Home.
* Writing, updating, and maintaining clients' Care Plans
* Completion of documentation, referrals and returns where necessary
* Liaising with necessary internal / external stakeholders, specialist and agencies keeping them up to date with progress of applications / referrals.

About The Knole
The Knole is a Registered care home for men with multiple needs, including substance misuse, mental health needs or a learning disability. It is located in Cheltenham. The project is a Forensic Step-Down facility with a small friendly team of 16 and we have 15 clients.
