Short term services are recruiting for a Support Worker to join our team on a part-time (36 hrs) permanent contract where you will provide care and support to people in the community with a focus on maximising independence and delivering a high standard of care to a variety of people, including end of life care.
Following a period of acute ill health or hospitalisation, you will undertake short term care as defined in care & short term service plans or delegated tasks from Therapist/Nurses, and contribute to the assessment process and ongoing monitoring and review of people's goals, reporting any changes to a person's medical condition.
This is an exciting development of services to help prevent avoidable hospital admissions, working alongside intermediate care teams and other community teams.
You will be working as part of a team, including lone working within the community, across Torbay and South Devon to provide integrated health and social care, working closely with other organisations.
Responsibilities include:
1. Provide support to service users with personal care, domestic care, social needs, and clinical needs in accordance with the care plan and within the limits of own competencies and responsibilities.
2. Observe and evaluate the service user's response to the care plan and communicate changes in need and/or circumstances to the appropriate member of the service in a timely manner.
3. Provide a flexible service which will include shift patterns between 0800 hrs and 2200 hrs, 7 days a week.
4. Provide reports on service users using the service IT system, while complying with the Trust Policies and Procedures.
5. Contribute to a safe working environment for self and others and report situations accordingly in line with Trust Policies and Procedures and other partner organisations.
6. Undertake training relevant to the post and in line with personal development and review.
7. During times of reduced work, provide support to other teams linked with the service.

Key tasks include:
1. Assist and promote independent social and living skills including the provision of a range of tasks, such as assistance with mobility, personal care, bathing, dressing/undressing; continence, nutrition, hydration, and prompting, monitoring and administration of medication where required, in line with local policies.
2. Therapeutic moving and handling skills – often requiring prolonged physical effort and frequent use of a wide range of moving & handling equipment.
3. Undertake specific clinical tasks as delegated once relevant training has been completed and deemed competent, including baseline observations and investigations, simple wound care, and dressing management.
4. Follow goal plans and use observational skills to evaluate and modify interventions to participate in the reablement of people, engaging them as an active part of the process.
5. Fit and adjust community equipment to people and their environment.
6. Teach people the correct use of specialist equipment.
7. Provide feedback to team or clinical leads regarding outcomes, concerns, progress, and deterioration related to the achievement of set goals.
8. Form professional relationships and communicate effectively with service users, team members, families, and carers, managing any barriers to effective communication.
9. Ensure that patients, relatives, carers, and professionals are kept fully informed of progress following intervention.
10. Contribute to ensuring a safe working environment for self and others and report on situations of potential risk, in line with local policies and procedures.
11. Only undertake work within own competence, seeking advice and supervision from Team or Clinical Lead as necessary.
12. Be aware of and act upon local health and safety policies, promptly recording any accident, incident, or other untoward event involving self, patients, or other staff.
Ability to travel efficiently around the Bay is important in order to carry out your duties (e.g., Full driving licence and vehicle).
