Full time 37.5 hours and part time 18.75 hours
We are seeking a highly motivated and enthusiastic person to work as part of the Medicine and Urgent Care Navigation Team to monitor patients through pathways, monitoring progress and identifying potential breach patients - taking appropriate action to avoid these.
The post will involve working as part of a team within the Gastroenterology services to ensure that all administrative functions for patients from the time of GP referral to the time of discharge/treatment are coordinated in line with specific care pathway instructions/targets to facilitate the smooth running of the patients’ journey.
This demanding post requires excellent interpersonal and organisational skills, and the ability to communicate at all levels. A flexible approach is essential, as is the need to be self-motivated with the ability to work under pressure and prioritise tasks/objectives.
