Job summary An exciting opportunity has arisen for an individual to join our fast-growing, friendly team as a Advanced Nurse Practitioner for 22.5 hours per week. We are looking for a friendly, professional individual that strives to go the extra mile for their patients and you will work as part of a large multi-disciplinary team. Main duties of the job The post holder will demonstrate critical thinking and be a committed, experienced nurse who, acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care for our patients. The post holder will communicate effectively and work collaboratively with the general practice team and with our colleagues within the Integrated Care Centre to meet the needs of patients, supporting the delivery of policy and procedures. About us Cardigan Health Centre has approximately 10,400 patients. We are a GP training practice and we are based within the new Cardigan Integrated Care Centre. We strive to deliver the highest quality healthcare to our patients, through continuous training and development of all staff. We are a short walk away from Cardigan town and only a few miles drive away from many beaches We work alongside other service providers in the Integrated Care Centre, which include MIU, Xray, Physiotherapy, Sexual Health, Outpatients and District Nurses. Our team is made up of 4 GP Partners, 2 Salaried GPs, 3 ANPs, 3 Nurses, HCA and a Phlebotomist, who all work alongside our large Reception and Admin. We currently use Vision clinical system and we are migrating over to EMIS Web in July. Full training will be provided on both systems.
