Job Description - Support Coordinator

Want to be part of this? We’re currently recruiting for a Support Coordinator to join our Sheltered Housing team.
What you’ll achieve in the role:
The overall purpose of this role is to effectively manage a portfolio of sheltered accommodation schemes, to promote independence and facilitate support for residents.
Key responsibilities include:
1. Carrying out assessments for new customers, process terminations of tenancies, and set up and establish tenancies on Raven’s IT system.
2. To be in regular contact with customers, including undertaking visits, calls, emails, and texts, to provide support and enable independent living.
3. Work closely with customers to assess their individual needs and develop support plans, whilst promoting dignity, confidentiality, and individual choice.
4. To maintain contact records and general administration for each tenant.
What you’ll bring to Raven:
For this role, we are looking for someone with the following:
1. A good understanding of the housing support needs of older people.
2. The ability to effectively manage challenging behaviour and demonstrate ability to adapt style to suit customer needs.
3. The ability to use modern communication technology and IT.
4. Excellent interpersonal and communication skills, with the ability to build strong relationships.
A full, clean driving licence with access to own vehicle.

DBS clearance will be required for this role.
