Phyllis Tuckwell provides supportive and end oflife care for people living with an advanced or terminal illness. PhyllisTuckwell is based in Farnham, Camberley and Guildford and serves a populationof 550,000 across West Surrey and NE Hampshire.
We have an opportunity for a Clinical Nurse Specialist working within one of three localitieswith flexibility to work across our catchment to respond to patient needs. The three localities are: Guildford, and Waverley,Surrey Heath and Farnham or North East Hampshire.
Salary £49,400 WTE based on hours per week
Full and part time opportunities available
Full time hours - worked across 4 days of hours or 5days of hours on a seven day rota.
Part time hours - worked across 2 or 3 days of hoursor 4 days of hours on a seven day rota.
Location: Farnham or The Beacon Centre at Guildford.
We are looking for an experienced senior nurse withexpertise in palliative and end of life care, clinical leadership andpartnership working, to join our community team to provide specialistpalliative care to patients, their families, and carers in the community.
Main duties of the job
The Clinical Nurse Specialist role will beresponsive to need, demonstrating advanced communication skills, advance careplanning, symptom management knowledge, caseload management, as well ascritical thinking and decision-making skills.
The role will include working in our Advice andReferral Team which provides a single point of access for referrals, advice,and support for multidisciplinary team working around the patient.
The successful candidate will work as part of amulti-professional team and will provide responsive care as well as proactivelyworking alongside patients and families to support them to live well.They will embrace opportunities to advance clinical practice, be willing toshare their skills and knowledge by educating others and be committed tocontinuously improving services. A commitment to further develop ownknowledge and clinical skills to improve patient care is essential including awillingness to undertake MSc modules in Advanced Clinical Practice.
