The Intensive Support Team (IST) has an exciting opportunity for a skilled, passionate, and dynamic Learning Disability or Mental Health Nurse to work creatively to make a real difference to the lives of Adults with a Learning Disability.
We are looking for individuals who share our strong value-based approach and have an understanding of the support needs of people with a Learning Disability and Autism, who are at risk of an acute hospital admission or placement breakdown.
This post is for a Band 6 Lead Nurse, working 35 hours per week on a permanent contract and will be based at Ash Green LD Centre, Chesterfield, covering the North Derbyshire geographical area.
Please be aware that the Intensive Support Team cover 7 days a week, 52 weeks a year; between the hours of 8.00am and 8.00pm. A variety of shift patterns are available, including 8.00am-4.00pm, 12.00noon-8.00pm and long days. Exact details can be negotiated with the right candidate.
Main duties of the job
The IST is part of the multi-disciplinary Derbyshire Adult Neurodevelopmental Services. We work closely with a variety of stakeholders - Individuals, their families and carers, colleagues in Adult Care, Primary Care and Specialist Care Providers. Our focus is on the individual and we aim to provide responsive, intensive and personalised care, to avoid inappropriate hospital admissions. We work to maintain individuals in their own home and engaged in their own community, following the aims of the NHS 'Building the Right Support' Action Plan.
Job responsibilities
As a Lead Nurse within the North Derbyshire Intensive Support Team you will be an essential member of our senior leadership team, working collaboratively with colleagues to deliver specialist, flexible, responsive and intensive support for adults with learning disabilities considered at increased risk of a hospital admission under the MHA 1983.
You will work as part of a multi-disciplinary team made up of skilled nurses, therapists and support workers. You will work with a wide range of other agencies and stakeholders to provide bespoke support for people experiencing a challenging period, often for a complexity of reasons. You will be expected to support a variety of students during training, placements, and insight visits.
You will have specialist knowledge and experience in a variety of areas, including communication, behaviour, mental health issues and other common conditions such as Autism. If you can demonstrate excellent communication skills, flexibility and are able to plan and deliver high quality, person-centred, evidence-based clinical care, we would very much like to hear from you.

* RNLD/ RMH Qualification
* Nursing & Midwifery Council Registration
* Evidence of successful completion of post registration education
* Experience of management or leadership of teams
* Mentorship Qualification/ Standards for Student Supervision & Assessment

Experience & Knowledge
* Experience working with people with learning disabilities OR equivalent transferable skills

* Knowledge of Transforming Care, MHA, MCA and Positive Behaviour Support.
