Black Country Healthcare NHS Foundation Trust
Due to Community Transformation, we have multiple new, innovative fully-funded vacancies (either FT or PT) for motivated Band 6 Mental Health Nurses to join our Primary Care Mental Health Team in Sandwell.
You would be based out of an office (Quayside House, Oldbury) with regular Primary Care/GP or Home visits, so having a car is essential. As this is a new service, this is your chance to be part of, and shape our community transformation within Sandwell!
The successful candidate will join our Primary Care Mental Health (PCMH) service, which bridges the gap between NHS Talking Therapies and Community Mental Health Services. The service provides triage, assessment, signposting, referral, and brief NICE guidance interventions delivered through a person-centered and recovery-focused model.
We are excited to be developing our integrated community services and look forward to welcoming you to a hardworking and dedicated team. You can find out more about these transformative developments on our website.
The successful candidate will work closely with Primary, Secondary, and Specialist Care Teams, ensuring that individuals with mental health problems are stepped up/down and receive care and treatment appropriate to their individual needs.
If you're someone who is innovative, proactive, and passionate about providing high-quality holistic care, then don't miss your opportunity to apply for this exciting role.
