Position: Care Team Lead
Responsible to: Care Manager
Purpose of Position
To support the Care Manager in the day to day running of the service, ensuring compliance with all requirements of CQC, promoting a high-quality service which encourages independence and choice to our customers enabling them to reach the optimum level of independence whilst living in their own home.
To monitor the safety and the quality of the service in line with internal policies & procedures and external regulations and legislation.
To supervise & monitor staff in meeting the personal care needs of customers in a way that respects the dignity of the individual and promotes independence.
Principal Responsibilities
· To supervise, support and mentor care & senior care & support staff in a geographical area, undertaking observations and supervisions as required.
· To work a variety of shifts, on a rota basis to cover care visits and staff absence as required, ensuring the full provision of services.
· To organise and plan rotas to ensure that staffing levels meet the needs of the business.
· To act as the senior on-call on a rota basis, offering out of hours support to care staff, providing a handover to the manager as necessary, ensuring all care calls are covered as planned.
· As required support the service in the Manager’s absence, ensuring the full provision of the service is met, providing a handover to the Manager, as necessary.
· To carry out administrative tasks as required to ensure that customers’ information is up to date.
· To ensure that Medication Administration Records are updated accordingly.
· To ensure that stocks of medication in customers’ homes are at an appropriate level, recording and returning overstocks as appropriate to the pharmacy.
· To carry out audits and quality monitoring visits to customers’ homes and reporting findings back to the Manager for further action.
· To assess, reassess and review customers' care needs and to implement & update electronic care plans, as necessary.
· To carry out customers’ risk assessments and to implement risk management plans as required.
· To liaise with customers, their families, social workers, GPs, CPNs, District Nurses and other key people regarding any care packages, ensuring their awareness is raised to any change of needs.
· To work with external organisations to meet the needs of customers’ using the service, acting as an ambassador for the company to build effective working relationships.
· To act at all times to safeguard customers from any form of abuse and to report such concerns immediately.
· To ensure all Health and Safety procedures are implemented, monitored and reviewed as appropriate.
· To monitor that data protection laws are being adhered to.
· To participate in the recruitment and onboarding of care staff, ensuring a comprehensive induction and relevant training is undertaken.
· To support staff training ensuring that all staff are trained to the standards required and competent to provide high quality care.
· To perform such other duties as may reasonably be required.
