We have a fantastic opportunity for a motivated and forward-thinking person to join our team as a Case Officer (Adaptations).

Here in the HIA, we provide a range of services to older people and people with a disability, such as major and minor adaptations, and compliance on stairlifts, which all help to keep our residents living as independently as possible in their own homes. The primary purpose and role of our Case Officer is to provide a professional and person centred support service to Adult Care service users and/or their representatives whilst accessing adaptations to their homes.

Some of the day to day duties you will be involved with have been outlined below:
1. To manage a caseload of adaptations following an Occupational Therapy assessment, from referral through to completion of the work and payment of the grant.
2. To develop professional relationships with service users and other people involved in the process to ensure the smooth delivery of the adaptation.
3. To identify where a service user may be entitled to additional benefits, grants or services and sign post as appropriate.
4. To act as the first point of contact for the service user throughout the process as their named contact.
5. To collate all necessary permissions, plans, costings and reports to support the application of the adaptation for decision.
6. To ensure that payments are made for completed adaptations correctly and in a timely manner.
7. To undertake the financial means test and let the service user know how much they will need to contribute to their adaptation.
8. To produce and maintain, detailed and accurate records/ case notes in relation to your allocated cases, ensuring they are person-centred and a true reflection of discussions/ agreements.
9. Part of the role will be to visit service users in their homes to assist with the completion of forms and discuss issues and questions they may have about the process.
