Join Our Team
At Mountain Valley Hospice, we provide compassionate, personalized care to patients and their families during life's most challenging moments. We are currently seeking a dedicated and skilled Registered Nurse (RN) Case Manager to join our team, delivering exceptional care in patients' homes, skilled nursing facilities (SNFs), and assisted living facilities (ALFs). As part of our team, you'll collaborate with nurses, social workers, chaplains, and providers who share a deep commitment to high-quality care and support. What You'll Do
* Provide expert clinical care to patients facing serious or terminal illnesses, ensuring comfort, dignity, and quality of life.
* Assess and develop individualized care plans, adapting as patient needs evolve.
* Educate and support families, helping them navigate care decisions and understand symptom management.
* Coordinate care with an interdisciplinary team, including physicians, social workers, chaplains, and CNAs.
* Ensure accurate documentation and timely communication regarding patient conditions and care plans.
Qualifications
* Valid Registered Nurse License
* Exceptional interpersonal and communication skills, with a compassionate and patient-centered approach.
* Strong ability to work both independently and as part of a team, with excellent time management skills.
