We have an exciting and rare opportunity to join the Southeast Staffordshire Rehabilitation Team based at Samuel Johnson Hospital, Lichfield. The team consists of Physiotherapists, Occupational Therapists, and Specialist Nurses providing intervention to adults within the Burntwood, Lichfield, and Tamworth locality.
Referrals to the service cover a range of conditions including traumatic brain injury, Stroke, Multiple Sclerosis, Parkinson’s, MND, other neurological conditions, spinal cord injury, falls, orthopaedic conditions, amputations, hip and knee replacements, arthritis, fractures, early onset dementia, and multiple pathologies.
You will be required to:
* Work on your own as a registered practitioner within the established multidisciplinary team, undertaking a Community Physiotherapist role for the South East Staffordshire Community Rehabilitation Service.
* Assess, identify, and agree goals and treatment plans using diagnosis and evidence-based treatment programmes to meet a wide variety of patient needs in the most appropriate location in the community e.g. patients’ own homes, clinics, and care homes.
* Support the team in providing training to carers, care agencies, and other external partners on an ad hoc basis at the request of Operational and/or Clinical Leads.
* Hold responsibility for your own caseload and work without direct supervision, undertaking regular supervision and training of rehabilitation assistants and supervising, educating, and assessing the performance of Physiotherapy students.

Main duties and responsibilities:
* Undertake comprehensive assessments using verbal and non-verbal communication skills, highly skilled physical examination, and manual assessment techniques. Provide accurate clinical diagnosis, agree physiotherapeutic goals, and the physiotherapy treatment plan with the patient/carer.
* Work to CSP and HCPC code of professional conduct and within guidelines. Maintain a good working knowledge of national and local standards and monitor own and others quality of practice.
* Maintain own competency through Continuing Professional Development activities and maintain a portfolio reflecting personal development and meeting HCPC standards.
* Develop current knowledge of evidence-based practice, developing specialist knowledge of conditions and patient types through in-service training, self-directed learning, tutorials, peer review, and external courses.
* Hold responsibility for own specialist caseload and a defined area of service/patient type, working without direct supervision as an autonomous practitioner.
* Continuously assess and reflect on current practices and lead on changes in clinical and working practices to ensure that recognised ‘best practice’ is implemented.
* Demonstrate a sound understanding of clinical governance and risk management and apply it to work situations e.g. lone working, manual handling, incidents, and accident reporting.
* Participate in clinical supervision, peer support, staff appraisal, and personal development plan.
* Supervise, educate, and assess the performance of Physiotherapy students, ensuring the standard of practice and teaching meet the standards set by the degree level qualification.
* Occasionally transport patients and carry equipment using your own car.
Systems and equipment:
Input data on patient recording systems (currently Rio) within the timescales specified in the record-keeping policy. Adhere to policies and procedures ensuring patient safety e.g. risk assessments, vulnerable adults, and infection control.
Decisions and judgements:
Work autonomously with responsibility and accountability for planning, prioritisation, and management of your own caseload and workload, including allocation of appropriate duties to assistants, students, and admin staff where appropriate. Formulate accurate prognosis and recommend the best course of intervention, developing comprehensive discharge plans. Implement, evaluate, analyse, and adjust treatment programmes according to patient and carer needs, which may be highly complex.
Communication and relationships:
Work closely with South East Staffordshire Community colleagues and in partnership with Primary Care networks, to provide a service in an integrated manner. Act as a source of spontaneous and planned clinical expertise, giving appropriate advice and guidance for the patient, carers, and other professionals within health, education, social services, and the voluntary and private sectors, within safe practice guidance. Provide and receive complex information relating to patients’ diagnosis, treatment, and long-term rehabilitation outcomes to patients, their carers, and the multidisciplinary team.
