LOCATION OF POSITION: MDH Dorchester County Health Department 3 Cedar Street Cambridge, MD 21613
Main Purpose of Job: The main purpose of this full-time position will be to support the goals of Connecting For Success by providing direct skill building and support services to incarcerated and recently released parents and their families. This is accomplished through assessments, reentry planning and goal setting with program participants, linkage to community-based support services, and facilitation of skill building group sessions using appropriate curriculum. This position will meet with program participants in the local correctional facility, in the community, and/or home setting. This position will also make referrals to and collaborate with community partners and agencies. In addition, this position will require daily independent community-based and jail-based service but will also work as a team with other professionals to achieve coordinated service delivery that benefits the program's participants and ensures the success of program goals.
MINIMUM QUALIFICATIONS:
1. Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
2. Experience: One year of administrative staff or professional work in a health care setting.
Notes:
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
DESIRED OR PREFERRED QUALIFICATIONS:
1. Experience working inside a correctional facility.
2. Experience providing outreach and reentry planning to incarcerated and/or recently released adults.
3. Experience with and/or knowledge of community resources in Dorchester County.
