Responsibilities:
* To Conduct comprehensive assessments of adults in need of social care intervention, taking into account their psychological, cultural, and emotional needs.
* Collaborate with healthcare professionals to develop effective discharge plans. Ensure that service users are safely discharged and have the necessary support in place.
* Provide support and guidance to service users to help them regain their independence through rehabilitation.
* Manage a caseload of service users, regularly reviewing and updating care plans as necessary
* Work as part of a multi-disciplinary team, maintaining effective communication and cooperation with colleagues from various disciplines.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
