We are looking for a dedicated Community Connector to join our recovery and aftercare team in Gloucestershire. This role involves supporting clients who have completed treatment and need help to further develop their recovery capital. You will act as a bridge between structured treatment and aftercare, ensuring clients receive compassionate and collaborative services. Your main responsibilities will include conducting individual assessments, developing personalized recovery plans, providing crisis intervention, and assisting clients in accessing necessary services such as mental health support, housing assistance, and employment resources. You will also facilitate participation in mutual aid groups, provide information on addiction and relapse prevention, monitor progress, advocate for clients, involve family members, and support volunteer activities. Additionally, you will ensure all safeguarding concerns are reported, support the delivery of recovery coaching and peer mentoring programs, and coordinate recovery-focused activities.
Key Responsibilities:
* Conduct individual assessments and develop personalised recovery plans.
* Provide crisis intervention and assist clients in accessing necessary services such as mental health support, housing assistance, and employment resources.
* Facilitate participation in mutual aid groups and provide information on addiction and relapse prevention.

* Coordinate recreational activities and community events.
Key Requirements:
* Driving licence with full access to a vehicle and willingness to travel (essential)
* Experience of group facilitation (desirable)
* Experience of working in substance misuse support (desirable)
* Strong IT skills (essential)
