We are currently recruiting for an Assistant Practitioner to join our community Paignton Health and Well-Being Team.
To work as a Assistant Practitioner within an integrated team, will result in your working across the following services: Community Nursing, Intermediate Care and Community Matrons.

Main duties of the job
You will deliver high quality care to the housebound patients of Paignton, and work as part of multi-disciplinary team in partnership with primary care and secondary care.
