One of our leading NHS clients is looking for experienced mental health nurses to cover the shifts in the community hospitals in York and Humber. The right candidate will be enthusiastic and a highly organised mental health professional who is able to provide high quality nursing care and assessment.
Description:
1. The postholder will work closely with the team of doctors and be involved in carrying out initial assessments for patients, ensuring that prescribed medication has reached the patients and follow up to monitor the response to the medication.
2. They will monitor side effects and complete physical checks such as blood pressure screening.
3. They will also support to assess/determine what the individual's mental health needs are and identify potential risks to inform their care plan and develop a collaborative response to needs.
4. They will support with follow up to ensure patients are stable on their prescribed medication and carry out discharges in collaboration with the MDT.
5. Have a weekly allocation of patients and carry out their initial assessments.
6. Hold a caseload of patients and managing their access to anti dementia medication and monitoring of their response to the medication.
7. Demonstrate the skills to professionally communicate complex information to users and carers regarding bio-psychosocial problems in an empathetic and supportive way.
8. To liaise with professionals/ agencies and ensure the patient receives continuity of care/ service through clear and timely communication with all involved in their care.
9. To help in information gathering in line with the team's needs and record these on the patient records.
10. To attend MDT meetings as required for MAS (Memory Assessment Service).
Skills & Experience:
11. Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse
12. Mentorship qualification
13. Experience of taking responsibility for a ward / clinical unit.
14. Experience of engaging / supporting clinical research projects
15. Experience of supporting practice development in clinical areas
16. Able to provide quality care that is responsive to service user's needs, without close supervision
17. Resilient: able to cope with difficult interpersonal situations.
18. Self-aware, self-confident and intrinsically motivated to do a good job and to motivate others
19. Forms very effective working relationships with colleagues
20. Advocates for all individuals, particularly those with protected characteristics; promotes equality and diversity
