We are looking for an 8 session ANP to work Monday, Tuesday, Wednesday, and Friday.
The Advanced Nurse Practitioner (ANP) plays a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate. The ANP operates within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners.
Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, and advise patients across various settings including face-to-face, telephone video, and e-consultations, ensuring the use of evidence-based practice. The ANP will also lead in the development of new services and pathways to improve the quality of care and patient outcomes. This role requires dynamic clinical leadership and active participation in practice research activities to promote a culture of learning and continuous improvement within the practice.

We are a semi-rural practice with 2500 patients situated on the border of Gateshead and County Durham, part of Gateshead Outer West PCN. We are a historic mining town recognized as a Deep-end surgery due to the socioeconomic structure of the population. We have onsite physio on a Monday, rapid access to counselling services, and PCN pharmacists.

* Prescribing: Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.
* Home visiting: Undertake home visiting as and when required, including triage of home visits.
5. Leadership and Guidance:
* Mentorship: Provide mentorship and supervision to nursing staff, trainee advanced practitioners, and other healthcare professionals to enhance team competencies.
* Service Development: Lead the development and implementation of new clinical services and pathways, improving processes and patient care.
* Professional Development: Engage in continuous professional development to keep abreast of medical advancements and to further develop clinical skills.
6. Research and Quality Improvement:
* Research Participation: Participate in clinical audits and research activities to contribute to the evidence base for practice.
* Quality Assurance: Implement quality improvement initiatives and ensure compliance with healthcare standards and best practices in patient care.

* Training Provision: Deliver training sessions to healthcare staff on clinical skills, advanced practice techniques, and case management.
* CPD Activities: Facilitate and participate in Continuous Professional Development (CPD) activities within the practice.

* Record Keeping: Maintain comprehensive and accurate patient records using Emis.
* Meetings: Attend and contribute to team meetings, providing updates and insights on patient care strategies and outcomes.

Person Specification
Experience
* Clinical Skills:
o Extensive experience in managing complete episodes of clinical care.
o Advanced clinical skills in a primary care setting with proficiency in using clinical diagnostic tools and conducting complex clinical procedures.
* Communication and Interpersonal Skills:
o The ability to convey complex information to patients and team members effectively.
o Strong interpersonal skills with the capability of working collaboratively within a multidisciplinary team.
* Leadership and Mentorship:
o Experience in a leadership role.
o Demonstrated ability to lead service improvement initiatives and develop new clinical pathways.
o Competence in using Emis.
* Regulatory Knowledge:
o In-depth knowledge of current national health policies and clinical guidelines relevant to primary care and advanced practice.
Qualifications
* Educational Background:
o Masters degree in advanced Practice, or equivalent post-graduate qualification, or full accreditation or e-portfolio.
o Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate.
