Assessment and Engagement Coordinator - Located in Lewisham
Hybrid Role (This role is a mix of WFH and working from our Dartmouth Road (SE23) service) - Initial 12 month FTC. Please note: Interviews for this position will be held face to face.

Humankind Primary Care Recovery Service (PCRS) in Lewisham is looking for a full-time Assessment and Engagement Recovery Coordinator to join our friendly team of Recovery Coordinators.
The ideal candidate will have good knowledge of different illicit substances including alcohol, be caring, compassionate, empathetic, and passionate about helping others. Good time management and communication skills (both written and verbal) are also very important.
The aim of this role is to engage and retain people in treatment within the borough of Lewisham.
Duties will include but are not limited to:
* Being the first point of contact for all new PCRS Lewisham service users, building rapport and understanding their struggles with substance misuse.
* Contacting new referrals and arranging triage assessment appointments in a timely manner (within 3 days of the referral).
* Completing initial and comprehensive assessments and updating our database system SystmOne.
* For alcohol clients, completing SADQ and Alcohol audit.
* Booking clients for healthcare and alcohol assessments where appropriate.
* Completing DBST.
* Liaising with referrers if clients DNA their triage appointments/disengage.
* Holding on to all triaged Tier 3 clients for a maximum of two to three 1:1 KW sessions following their triage assessments.
* Ensuring that all Tier 3 service users have updated Risk Assessments, Care Plans, and TOPs added on SystmOne prior to their transfer to their allocated Recovery Worker.
Please note - this position does NOT require prison clearance as this is a community-based role.

Salary Period: Per Annum
Vacancy Salary Range: £26,250 - £34,650
