As part of community transformation, an exciting role has been developed between TEWV & Tees Valley Primary Care Networks (PCNs).
We are looking for enthusiastic Community Associate Practitioners to work within the Tees Valley. The post holders will cover the Middlesbrough, Hartlepool, or Stockton Localities.
The role is to ensure that the physical health needs of SMI patients are met. The practitioner will be expected to link in with individual GP practices to identify patients outstanding for physical health checks.
An assertive outreach approach will be adopted to engage patients on the SMI register to have their physical health investigations carried out. A kit will be provided to ensure that physical investigations can be carried out, at times, outside of a clinical environment. Full training on the kits will be provided.
Time will be shared between TEWV & the PCN, meaning the new role will be carried out alongside the duties of an Associate Practitioner within TEWV.

The successful candidate will be able to work as part of an integrated Community Mental Health Team and Primary Care Services. The role will involve working alongside the Leadership Team and Care Coordinators towards providing recovery-focused interventions with service users and supporting carers.
Managerial and clinical supervision will be provided to support the successful candidate in managing their caseload.
There are varied opportunities for training and development, with clinical development needs being identified and monitored through appraisal.
The successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into this position on Band 4.

Person Specification
Qualifications
* Foundation Degree in Health and Social Care
* Key skills in literacy, numeracy, and ITQ Level 2 (or equivalent)
Experience
* Previous experience of working with people with mental/social health needs or learning disabilities.
* Previous experience as a Band 2/3 HCA or equivalent in a care environment.
* Experience of working in the community mental health team.
Knowledge
* Awareness of legal frameworks including Mental Health legislation, The Care Act 2014.
* Evidence-based knowledge of best practice in caring for adults with enduring/complex mental health difficulties.
* Willing to undertake learning in specific areas which are relevant to the role, e.g., challenging behaviour, personality disorders, psychosis.
Skills
* Time management and ability to prioritise effectively.
* Competency in record keeping (within agreed timescales).
* Effective verbal and written communication skills.
* Ability to address and resolve conflict.
