The Out of Hours Community Nursing Team across Cambridgeshire is currently offering opportunities for permanent contracts working full time & part time within CPFT across the county working in Cambridge Huntingdon, Peterborough .
We provide a service 7 days a week, working closely with other community services and PCN's to provide high standard holistic care to the population of Cambridgeshire .
All staff will be expected to work the following shift patterns to support the service over a 7 day period
Twilight Shift 1800-2400
Night shift 2145-08:15

Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
As a Community Staff Nurse, you will be working as an autonomous practitioner within the out of hours countywide team.
You will provide high standard of nursing care and interventions within the patient's own home, with support from team leaders.
The patient will be at the centre of the care being provided, ensuring patient and carer participation in decision making.
You will be accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients within professional guidelines. Patient needs will be identified through holistic nursing.
You will have opportunity to provide support and education to peers, new staff and students.

1. To be professionally and legally accountable for all aspects for your own and delegated work.

3. To use evidence-based practice, to ensure clinical care is effective and appropriate.
4. To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
5. Contribute to risk assessments and health and safety assessments and including immediately reporting any changes/newly identified risks
6. To effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication, hearing loss, diminished sight, depression, speech problems, cognitive impairment, behavioural problems and pain.
7. To work effectively within the Out of Hours Team structure and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews.
8. To ensure that accurate and timely written records are kept which comply with the Trust policy and NMC guidance, reporting on any issues as appropriate.
9. To inform/update all members of the multi-disciplinary team, service users and appropriate others, of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient.
10. To ensure that medications are stored and recorded according to the rules relating to the drugs policy of CPFT.
