Clinical Nurse Specialist Palliative Care

An opportunity has arisen to join our Palliative care clinical nurse service. A 37.5 hour post has become available as a result of retirement from the role within the team.
The Band 7 CNS role is located in Y Bwthyn, our palliative care unit based on the Royal Glamorgan Hospital site. The role provides an outreach service to patients with palliative care needs predominantly in the hospital at present but also in the community as part of a rotation.
While working as part of the 7 day roster, the service provides support to both the hospital and community area. The service supports the population of Rhondda Cynon Taf and the hospital population of Royal Glamorgan Hospital.
The specialist palliative care service is delivered within teams on multiple sites across Cwm Taf Morgannwg. There may be a need for post holder flexibility in providing support to cover the CNS role across other sites, depending on patient needs.
Main duties of the job
The position involves directly supporting and advising patients with palliative care needs in a hospital and community setting.
Multidisciplinary team working to lead education and to provide support and guidance to colleagues about the specialty is key. The core hours are Monday to Friday 9-5pm, but with participation in a 7 day roster with the same working hours. The role will require giving advice to other health care professionals, patients, and families, potentially visiting patients in the hospital and community setting at weekends.
The successful applicant would be responsible for managing a caseload of patients with specialist palliative care needs in collaboration with the wider MDT, participating in education, audit, and providing leadership regarding the specialty.
You will need to be enthusiastic about the specialty, demonstrate experience of caring for patients with palliative care needs, and be able to meet the travel requirements.
The Specialist Palliative Care Team in CTMUHB is a multi-professional team providing a clinical, advisory, and educational service across CTMUHB. The team is situated in locality bases which each have a SPC inpatient unit and house services that provide outreach services to both community and hospital patients.

Qualifications
* UK NMC Registered Nurse
* Qualification in palliative care at Degree/Diploma or equivalent skills, knowledge, and experience, supplemented by specialist training, experience, and short courses to Master's Level equivalent
* Teaching and assessing qualification
Skills
* Assessment and symptom management skills
* Proven ability of practice development and innovation
Experience
* Significant post-registration experience, with experience of caring for patients with palliative care needs
* Bereavement Care; Audit; Community nursing
