About the Role
As a Change Mental Health practitioner, you will work directly with service users who require immediate or ongoing emotional, practical, or vocational support in order to deliver the best possible outcomes. You will work with service users to develop and review personal plans, although ultimate oversight rests with the Senior Project Worker. You will work closely with other members of staff to achieve outcomes as defined by your line manager. Within the area team, there is a clear line management structure which supports and shapes your practice. You will aim to contribute to an environment where people can relax, re-establish social skills, and enhance their quality of life.
Key Responsibilities
* Provide front-line support to individuals living with dementia in Dumfries and support community activity groups.
* Develop and review personal support plans for service users.
* Facilitate family support groups and work with individuals on a one-to-one basis.
* Collaborate with other statutory and third sector agencies.
* Maintain community connections and network effectively.
Required Skills
* Good communication skills and a caring attitude.
* Willingness to learn and participate in training (including SVQs).
* Relevant qualifications and/or experience of working with people with dementia or substance use issues.
* Ability to work independently and manage your workload efficiently.
