An exciting opportunity has arisen for a CAMHS Crisis & Home Treatment Practitioner within Solar. Solar provides specialist Child and Adolescent Mental Health Services for children and young people 0-19 years old in Solihull.
We offer generous supervision arrangements including both peer/individual and support from a vast and varied multi-disciplinary team, working closely together to offer high levels of experience and expertise within a friendly and professional atmosphere. We are looking for an enthusiastic, motivated addition to the team who can help us on our journey to offering outstanding care for young people and families within the Solihull Borough.
With exclusive training package through the Anna Freud National Centre for Children and Families for the CYP Crisis Pathway Workforce.
Main duties of the job
* Provide high quality assessment for patients referred into Solar, including risk assessment and management, formulation of need, information gathering, and offering advice and guidance to children and young people directly and those who support them (parents/carers, family and professional support networks e.g., school nurses, teachers, social workers).
* Manage a defined caseload and be a care co-ordinator within the service.
* Work with partner agencies (statutory and third sector) to ensure a coordinated approach to manage the care of children and young people in Solihull.
* Ensure the most relevant care pathway and care plan is devised and provide seamless transition into other Solar teams and practitioners as needed.
* Contribute to a reduction in avoidable presentations to A&E departments.

TRAINING AND QUALIFICATIONS
* Registered Mental Health Nurse/OT/Practitioner
* Degree/Diploma/Specialist training
KNOWLEDGE
* Relevant post-registration experience in mental health
* CAMHS Community Experience
