1. Puts quality at the heart of practice by delivering evidence based, individualised and personalised care through implementation of a holistic needs assessment, care planning and evaluation of all care interventions. 2. Plans own diary and manages own caseload; plans specialised nursing service provision, co-ordinates input from other specialised services i.e. speech and language therapy, hospice care services. 3. Applies high level assessment and consultation skills to diagnose issues early and ensure timely treatment and support. 4. Attends multi-disciplinary team meetings. 5. Data entry on IT systems/MS Database. 6. Undertake clinical assessments i.e. blood tests, urine infections, blood pressure, infection screening. 7.
