This is an exciting opportunity to join a well developed and vibrant Primary Care Network Pharmacy Team. As a teaching PCN, we also have trainee pharmacists working and learning alongside our own team. The main priority for our PCN Pharmacy team is to alleviate daily pressure on the GP surgeries for which we provide clinical services.
About us
Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. PCNs were formally ratified in July 2019.
West Leeds Primary Care Network (WLPCN Ltd) is a large PCN with a patient list of circa 70,000 patients around the Pudsey / Bramley area. We have a significant workforce in place which brings wonderful opportunities for in-house multi-disciplinary team working. In addition to our Pharmacy Team, we have Occupational Therapists, Social Prescribers, Paramedics, Dietitians, a range of nursing / HCA staff and more which means that the majority of any patient's needs can now be provided on their own doorstep.
Primary Care Networks will be required to deliver on seven national service specifications, with which our pharmacists will play a key role:
* Structured Medication Reviews and Optimisation
* Enhanced Health in Care Homes Service
* Anticipatory Care
* Supporting Early Cancer Diagnosis
* Cardiovascular Disease Prevention and Diagnosis
* Tackling Inequalities Locally
Job responsibilities
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to:
* Management of long-term conditions.
* Supporting care homes within the PCN with structured medication reviews.
* Working with colleagues to establish and develop good medicines management systems across the care homes sector.
* Assisting the PCN care homes team and wider MDT with care home-related medication queries.
* Liaising with care home staff as necessary to ensure accurate exchange of information and clarification of medication queries.
* Working effectively within the PCN Pharmacy and Care Home Teams across all practices within the PCN.
* Establishing and improving professional standards of care in the management of residents within the care homes by developing relationships with key stakeholders.
* Contributing to repeat prescription authorisations and reauthorisation.
* Actioning acute prescription requests.
* Addressing both the public health and social care needs of patients.
* Contributing to the achievement of QOF and locally commissioned quality improvement schemes.
* Undertaking clinical audit.
* Developing, promoting and implementing the high quality, evidence-based and cost-effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).
This list is not exhaustive and additional duties may be decided by the PCN Leadership Team and the Senior Clinical Pharmacist.
The post holder will be supported by existing pharmacists employed within the PCN and a clinical mentor.
The post holder will be required to enrol on the General Practice Pharmacist Training Pathway from CPPE (unless an equivalent qualification is already held and exemption is agreed by CPPE) and will also be required to enrol on a prescribing course if a non-medical prescribing qualification is not already held.
