The Continuing Healthcare Teamis looking to appoint a dynamic, self-motivated Case Worker (Team Lead) to leada small team of case workers who will provide effective and efficient liaisonbetween the CHC Clinical staff and care providers. This is an exciting new rolewithin the Continuing Healthcare Team which is a busy operational team withinGloucestershire ICB.
Main duties of the job
The Case Worker team will workalongside the team of Nurse Assessors and Complex Case Managers within a rangeof settings and will undertake the non-clinical case management of those inreceipt of NHS Continuing Healthcare (CHC), Funded Nursing Care (FNC) and othersources of health funding. This will include liaising with placements,monitoring of care provision and changes to presentation. The Case Worker Teamwill support the Clinical staff to ensure the care provision meets the needs ofan individual, maintain accurate records and support all aspects of work forthose individuals in receipt of health funding.
The post holder will haveexperience of working within a range of different healthcare settings. You willhave excellent organisational skills and be able to work independently andcreatively, using a problem-solving approach. It is essential that you aremotivated, with strong leadership skills, a team player and enjoy the challengeof working in a fast-paced environment. The successful candidate will need tobe able to work well under pressure, managing competing demands and prioritiseaccording to the requirements of the service.

Job responsibilities
Thepost holder will work alongside the CHC Nurse Assessors & Complex CaseManagers within a range of settings undertaking the non-clinical casemanagement of those in receipt of NHS Continuing Healthcare, Funded NursingCare and other sources of health funding from Gloucestershire Integrated CareBoard GICB. This will include liaising with placements, monitoring of careprovision and changes to presentation which may affect an individualseligibility. Work with the CHC Clinical staff and providers to ensureindividuals are receiving appropriate care provision and regular reviews oftheir health needs.
Thepost holder will be responsible for providing effective and efficient liaisonbetween the CHC Clinical staff and care providers.
Thepost holder will be expected to provide accurate and effective communicationswithin and outside of the CHC service, dealing professionally and discreetlywith enquiries from patients, relatives, carers, legal representatives, health& adult social care professionals and other organisations.
Thepost holder will undertake reporting and analysis of information to supportdelivery and development of the role.
Thepost holder will need to meet deadlines, collate data and maintain standardsrelating to the Key Performance Indicators and Quality.

Physical Skills
* Skills for manipulating information.
* Advanced keyboard skills, use of a range of software.
Qualifications
* Knowledge of administrative procedures, project management or information analysis.
* Knowledge of commissioning care provision and contractual agreements and the roles & responsibilities of agencies providing these.
* Awareness of a range of health service provisions.
* Working knowledge of Microsoft Office.
* An ability to maintain confidentiality and trust and an awareness of Information Governance requirements.
* An ability to pull together draft reports, data and other correspondence.
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area.
* Working knowledge of NHS/ASC systems including SystmOne, Caretrack, RIO & LAS.
* Previous experience in a similar position within the public sector.
* Skills for communication on complex information and administrative matters, requiring developed interpersonal and oral/ written communication skills.
