Job summary To work as a key member of the multi-disciplinary Anticoagulation Team by assisting in keeping our patients safe on their high-risk anticoagulation medication and always keeping the patient at the heart of everything we do. The post holder will contribute to the clinical care of patients who take anticoagulants, by providing a safe, high quality service. The role involves direct patient contact, face to face and via telephone, discussing anticoagulation, working with the patient to improve adherence and optimise their warfarin control. It includes performing point of care (POCT) INR tests and phlebotomy at clinics in the community and the hospital. It also involves administrative tasks which support the anticoagulation nurse clinics and the Monitoring Service including booking appointments, following up patients who DNA and communicating dosage information. This interesting and varied role requires flexibility, excellent communication skills and a great attention to detail. Main duties of the job Main duties include: use of anticoagulation software to manage our patients appointments, communicate dose advice, check medication adherence, be a first point of contact for patients and other HCP's and deal with queries. - You will facilitate safe discharge of inpatients back into the community, manage referrals, book appointments, follow up referrals, patients who DNA, supporting both the Anticoagulation Monitoring Service and the Nurse-delivered clinics. You will perform INR tests by point of care and phlebotomy in our community and hospital clinics. You will work flexibly to ensure clinics are always delivered and help organise clinic staffing The role may require to undertake domiciliary visits. You will support and help manage the self-testing programme. Training and supervision of support staff.
