Our Dudley Wide PCNs are looking for a Care Coordinator who will work across the whole PCN, supporting our patient population and their personalised care requirements, supporting the PCN with triage and successful care navigation.
The candidate will need a flexible approach in the delivery of the PCN's aims for patient centred care. The role will require movement around all of Practices so access to a car is essential.
Main duties of the job
1. Identify a person's care and support needs and explore their options to meet these into a single personalised care in collaboration with the patient's primary care team.
2. Provide coordination and navigation for elderly people and their carers across health and care services, alongside working closely with social prescribing link workers, occupational therapists, dietitians.
3. Help people to manage their needs by referring onto the wider integrated care team.
4. To make patients feel more empowered and actively engaged to maintain independency.
5. Maintaining a register of patients who are over 80 and / or housebound across the PCN.
6. Face to face annual assessment review of patient's social, physical and mental health needs.
7. Referring to other agencies depending on needs.
8. Participating in integrated care teams to ensure the co-ordination of care.
9. Be part of a PCN wide triage team, to support with capacity and access plans.
10. Support practices with Cancer screening programmes, supporting the PCN Cancer Lead with data collection and auditing.
11. Supporting practices with FIT testing and checking EMIS coding is correct.
12. Keep accurate time sheets and other data on the service.
13. Ensure observance and adherence to the Health and Safety at Work Act 1974.
14. Work to the agreed standards, policies and procedures and protocols of the Practices and PCN and be fully conversant with emergency procedures.
15. Carry out other duties as directed by the line manager/Practice Manager which are consistent with the duties and responsibilities of the post.
Person Specification
Qualifications
* 5 GCSE'S Grade C or above including Maths and English (or equivalent).
* NVQ Level 3 Business Administration or similar administrative Essential diploma.
Knowledge
* Knowledge of public health issues in the local area.
* Knowledge of health promotion strategies.
* IT skills including knowledge of EMIS, Office and Outlook.
Experience
* Experience working in a Primary Care environment.
* Experience of partnership/collaborative working and of building relationships with other team members/professionals across a variety of organisations.
* Ability to organise, plan and prioritise effectively to meet deadlines, including when under pressure.
* Working knowledge and experience of using IT systems e.g. word, excel, databases.
* Ability to read large amounts of information and extract the salient points, to analyse data and communicate findings.
* Ability to keep all information confidential and awareness of information governance.
* Experience of working in a multidisciplinary setting where excellent influence and negotiation skills are required.
* Experience working as a Care Coordinator.
* Experience of working with professionals in a healthcare setting e.g. primary care.
* Knowledge/familiarity with medical terminology.
