Job summary The LTHT Cancer Pathway is designed for adults, where there is GP suspicion of cancer. This role has been designed to assist in coordinating the care of patients from point of referral, through the diagnostic pathway to diagnosis. It will require the post holder will need to adapt to support the pathway as it continues to evolve over time. The Pathway Manager will provide comprehensive support to the Surgical teams within AMS through the co-ordination of primary and tertiary referrals and supporting patients throughout their diagnostic pathway. The post-holder act as a single point of contact for patients and their families and as a liaison point between the GP, and medical and Surgical teams. Main duties of the job The post-holder will work both directly and indirectly with patients and their carers using a range of IT systems to assist in coordinating the care of patients through their hospital journey. The post holder will assist in the preparation of telephone triage clinics, co-ordination of patient results and deal with actions following triage. The post holder will coordinate appropriate services to maximise the delivery of streamlined diagnostic pathways, thereby improving quality of care and experience for patients and their carers. The post-holder will support patients to access additional services during or immediately after their diagnosis (including support information and advisory services). The post-holder will work across management teams and organisational boundaries, whilst linking closely with the Cancer Centre and contribute to plans and developments. Escalation of Actions The post-holder will: Monitor patient pathways against the agreed timescales in accordance with the specified pathways.
