Job summary The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to make sure that people living with cancer can access the right support, at the right time. Main duties of the job Participate in patient and public involvement activities to promote the service including awareness and promotional events outside of the centre. Present / attend cancer conferences locally and regionally as required. Organise the day to running of the service, organising meetings, bookings rooms, responding to email enquiries. Ensure compliance with policies, procedures and clinical guidelines for self and others. Work with other Co-ordinators across the region to adopt best practice. Use databases and computer packages to initiate and maintain records and generate statistics for analytical purposes. Coordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people. Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit. The post holder will demonstrate excellent organisational skills.
