Job summary
Appleby Medical Practice is looking for an ANP (Part time or Full time) to join their team. The successful candidate must be enthusiastic and forward thinking to become part of the small friendly team, with a well-established culture of holding the patient and their family at the centre of all that they do. At Appleby this is a role in which you will work with the team taking this role forward whilst further developing yourself and the practice, alongside current ANP colleagues.
Main duties of the job
Tomanage daily surgeries comprising of face-to-face consultations, telephoneconsultations and telephone triage work. This will include visiting ofpatients either in their own home or in a care home environment.
Todevelop and implement specialised treatment plans and health/ wellbeing programmesto manage patients presenting with a range of acute and chronic medicalconditions.
Toidentify and support patients at risk of developing long-term conditions andproviding specialist care.
Tocollaborate with GPs, nursing staff and non-clinical support staff alike.
Providewound care and specialist clinics, collecting pathological specimens andinterpreting test results as well as referring patients to secondary/specialistcare.
Maintainaccurate clinical records, ensuring read codes are used effectively &maintaining chronic disease registers.
Chaperonepatients where necessary and intervene appropriately, supporting the team indealing with clinical emergencies.
Bean extended and supplementary prescriber, adhering to guidance and supportpatients in the use of their OTC or prescribed medicines.
Contributeto practice targets (QOF etc.), complying with guidance and to liaise withexternal services/agencies to ensure the patient is supported appropriately.
Delegateclinical responsibilities appropriately.
Supportthe clinical team with all safeguarding matters.
Understand practice and local policies forsubstance abuse and addictive behavior, referring patients appropriately

Person Specification
Experience
Essential
1. Experience of working in a multidisciplinary team in Primary and/or Secondary Care setting.
2. Experience of selecting and prescribing a wide range of medicines appropriately, including antibiotics and analgesia.
3. Provide basic and advanced life support.
4. Providing general clinical advice and wider health promotion.
5. Ensure safeguarding of vulnerable patients.

6. Experience working in both primary and secondary care
