Our 247 Neighbourhood Mental Health Centre Pilot has been commissioned by NHS England to run Mental Heath Services in these areas in a new way. We will have an open door and will support any adults who need our help. The service is compassion based, looking at the whole person and ensuring all aspects of their life are considered.
We are looking for enthusiastic, compassionate, driven individuals within this community, who understand the needs of people in the area and are driven to make a difference. You do need need NHS experience for this role - we are very keen to have people who have worked for local charitable, faith based and voluntary organisations.
The role will be fully integrated into our Multi disciplinary team, working alongside our nurse, medical staff, psychology etc. You will have experience in supporting others and an awareness of exiting supports and facilities in the local area. You will take responsibility for an identified caseload and provide outreach and community follow up to support service users through their recovery using low intensity psychosocial interventions.
• You will be involved in all aspects of mental health care, assessment, care and support and treatment
• Support and engage people a caseload of people with mental health problems and their families/carers in collaboration with Key Workers
• Support the team in partnering other agencies in the community.
• With training support people build or rebuild all aspects of life
• Ensure social inclusion and active citizenship for people with Serious/Severe Mental Illness (SMI)
• Identify individuals, especially young people, at high risk of mental health problems (prodromes, social adversity, social withdrawal, risky health behaviour, such as onset substance misuse, antisocial acts, suicidal thoughts etc) in the community and provide support alongside the MDT.
• Participate in delivering hospitality from the hub
• Be part of planning and delivering a range of activities as needed alongside colleagues but inside and outside of the hub
