This is an exciting opportunity to join the Community Physical Health Team as we work closely with our Primary Care colleagues to deliver health assessments for individuals with severe mental illness. The post is open to qualified Nurse Associates covering Sefton, Liverpool, and Mid Mersey areas.
The post holder will demonstrate excellent clinical and communication skills and be a team player working closely with the Community Physical Health team and Primary Care.
You will have a clear commitment to person-centred care and will be able to demonstrate this in your work with individuals, families, and teams. You will be expected to work under the supervision of the band 7.
The post holder will provide physical health assessments, health promotion advice in line with national guidance, and refer individuals to appropriate services based on their health assessment. The successful candidate will also work with the team to develop and deliver enhanced support to the identified patient group and monitor the impact of their interventions on patient outcomes.
Main duties of the job
1. Provide clinical assessment, screening, and intervention for the physical health needs of our client group.
2. Work with qualified nurses to meet the health outcomes of clients and provide person-centered intervention and holistic care.
3. Liaise with identified GP practices to ensure patients' needs are appropriately identified and met.

Detailed job description and main responsibilities
The following list is indicative as tasks and responsibilities will vary depending on the care setting:
1. Develop understanding of all elements of the nursing process and assist the registered nurse in ongoing assessment, planning, management, and evaluation of care.
2. Support individuals with all aspects of care, providing person-centered care and promoting health and independence.
3. Demonstrate safe administration of medicines in line with trust SOP.
4. Perform and record clinical observations.
5. Undertake clinical tasks including venepuncture, ECGs, and cannulation (following appropriate training).
6. Accurately record nutritional and fluid intake and output.
7. Ensure the privacy, dignity, and safety of individuals is maintained at all times.
8. Recognize changing priorities and seek advice from the Registered Nurse or other registered care professionals as appropriate.
9. Report and share information with registered nurses on the condition and responses of individuals.
10. Recognize safeguarding issues and report concerns to appropriate registered care professionals.
11. Assist in the assessment and management of risk in care settings.
12. Assist in the delivery of complex care as prescribed by the registered nurse.
13. Maintain understanding of caring for individuals with specific conditions.
14. Develop skills in coaching/teaching individuals and other staff.
15. Engage in reflective practice.
16. Demonstrate understanding of consent principles and the Mental Capacity Act.
Person specification
Qualifications
* Nursing Associate Foundation Degree from an approved programme
* Registered Nurse Associate and on the NMC register
* Additional post-registration qualification relevant to the field of practice
